It’s critical to broaden the scope so that your organization can recognize and correct underlying issues that may be contributing to larger workplace safety problems.

Not sure how to conduct a workplace incident investigation? When strategizing, the goal of an investigation should be to determine the following:

• Was a procedure or safety rule not followed? If so, why wasn’t it followed?
• If a procedure or safety rule was not followed, was the safety training inadequate and in need of revising?
• Was the root cause of the incident not previously identified?
• If the cause of the incident was previously identified, why hadn’t it been corrected previously?
